VA Misled 440,000 Veterans About Their Incomplete Enrollment Applications
IG said VA tried to fix failing to resolve 867,000 incomplete veteran health care applications by sending wrong information to 440,000 of them.
Of course, agency leadership was caught trying to purge the system prior to the agency’s release of the IG report. Luckily, whistleblower Scott Davis called out the agency last week.
The review included “867,000 pending enrollment records” where VA sent out “440,000 outreach letters that … identified and requested the wrong information” needed to complete the enrollment process.
Think VA really intended to fix this issue? If you think the answer is “yes” please explain how it could get so many things wrong.
High-Level Enrollment Findings
Here is what IG found:
VHA did not provide effective governance necessary to ensure oversight and control over the health care enrollment program at its medical facilities. VHA relied on and required medical facilities to establish step-by-step procedures for processing enrollment applications without implementing effective processes and structures to monitor those activities.
We determined that only 38 of 106 VA medical facilities sampled had local enrollment policies. In addition, individual medical facilities that did have guidance were permitted to adopt local practices that were inconsistent with national policies, such as delaying or preventing the processing of applications in certain categories.
The reliability of health care enrollment data could also be improved. VHA data systems did not have the capability to identify new enrollment applications or provide the basis for independent testing of timeliness or supporting documentation. We projected that only 197,000 of our universe of 427,000 enrollment records (46 percent) represented FY 2015 applications for enrollment.
Furthermore, only 67 of the 127 FY 2015 applications (53 percent) in our sample had the timestamp required to measure processing time. As a result, we could not make conclusions related to timeliness or supporting documentation.
These irregularities occurred because VHA lacked effective governance over the health care enrollment program. VHA lacked appropriate guidance necessary to ensure a standardized enrollment process nationwide. In addition, adequate formal oversight and monitoring did not occur and quality control reviews were minimal. Formal training was also not provided to eligibility and enrollment staff at VA medical facilities. Finally, VHA did not adequately monitor program effectiveness or ensure that accurate data were available for program transparency.
Due to the variance in program implementation, VHA did not have reasonable assurance that veterans would receive proper consideration or consistent and timely enrollment decisions at VA medical facilities nationwide. VHA also could not accurately report on performance nationally or make informed program-level decisions based on available data.
Enrollment Examples
Here are some specific examples IG highlighted in its report:
Salt Lake City VA
The Salt Lake City, UT, VA Medical Center (VAMC) had a written policy in place for 10 years that specifically stated that veterans were not to be loaded into VistA until eligibility for health care can be established. The veteran was essentially turned away if the application was incomplete. Enrollment staff would not enter the applicant’s information into VistA and the facility kept no record of the applicant’s intent to apply.
Puget Sound Health Care System
The Puget Sound Health Care System had a standard practice of providing applicants with information cards that identified resources for obtaining military information. Enrollment staff would not enter the applicant’s information into VistA and the facility kept no record of the applicant’s intent to apply. The applicant was essentially turned away and VA-initiated follow-up would not occur.
Minneapolis VA
Another facility, the Minneapolis Health Care System, delayed health care enrollments by not interviewing individuals who applied in person and then deferring the entry of the applicants’ information into VistA to a later date.
All applications submitted to the Minneapolis facility were organized in a file cabinet and processed in the order in which they were received. Enrollment staff would then enter applications into VistA at a target rate of four applications per hour. This processing method was used even if the individual applied in-person at the facility. Enrollment staff did not interview the applicant as required and did not enter the registration into VistA while the applicant was present.
[E]nrollment staff provided applicants a return mail envelope with instructions on how to obtain evidence of military service. The entire burden of follow-up was transferred to the applicant and no other contact was made by facility enrollment staff. Conversely, at the Tampa VAMC, enrollment staff tracked follow-up activities and contacted applicants up to four times via mail in a 90-day period. While the facility made aggressive attempts at follow-up, the facility shortened the codified allowance of one-year down to 90 days.
just out VA employee removed ? Scopes used for different procedures were not properly cleaned, used on many veterans, could spread disease. Report sent to Obama, IG no know patients effected, no laws were broken.
They got ride of the employee, but no law was broken, no veterans have been identified as being effected. Why get ride of the employee if no law was broken. Why do an investigation, write to the president and spew evidence and then comment, No laws were broken and not veterans were identified of getting sick.
Also they had nurses admit that they were using injection pens over and over on veterans to give shots, These pens were to be used one time only. No veterans were identified of adverse reactions.
The nurses admitting to using these Pens, committed a criminal act in my opinion and should be held accountable, arrested and fired and all veterans given shots by these nurses notified.
Shulkin”s response to veterans committing suicide and then stating they did it because they Knew the VA would take care of them ! That is just crazy talk, we all know that the veterans were doing this to bring attention to the LACK of care.

I noticed the air bags did not deploy in the videos shown to us. I also saw the videos of the car getting its windows smashed out by the mob BEFORE he backed up. I really have a problem with medications, how can anyone treat anyone by percentages? Oh we can’t fill prescriptions because we went over 36% and we can’t give any opioids at 37%. Here’s Motrin and a rope to chew on.
I’ve been fighting the Denver VA for 3 years. They tell me nothing is wrong with my knee, hips and back, even though I’m SC for them. So because fighting with them because they are insane! They are trying to say I have a personality disorder, never once been brought up anywhere. I finally get all my X-rays together, along with MRIs and new X-rays. The sent me to a pain management doctor and was all set to take my fentanyl away because I wasn’t hurt. The whole story is pitiful and unbelievable. Anyway they finally reviewed thing in front of me and did a physical exam, first done at Denver. All they ever done is read radiology reports and treat accordingly whether correct or not.
So I went from nothing wrong, to having 4 discs in neck with spurs and stenosis, t5 pinched nerve, 6 discs in lower back that collapsed on left side, bone spurs in both hips, arthritis both hips, torn labrums both hips, bursitis both hips, acl reconstructed left knee with torn meniscus, arthritis both knees, torn meniscus right knee, fractured left ankle repaired with fibula and tibia fused, arthritis both ankles, arthritis both great toes and my mental health crap.
So the look on doctors face was enough, pain meds will stay the as long as VA regs don’t change and will start the treatment plan I had in CT. The doctor I almost choked 4 weeks before actually put in my notes nothing wrong and copy and pasted the wrong radiology notes for proof of his assessment.
Fucking crazy, I hate to scream at people. There’s nothing wrong with my joints and I’m screaming, never mind examining me! Look at the scars for surgeries! Since the VA couldn’t perform surgeries or not near a VA and were done by Fee Basis and not in the computer? Nothing exists!! It states as problem list in my medical notes all these problems and fail to read and listen. I just have a personality disorder, now the truth and not one sorry just a fucking nightmare and I’m lucky I didn’t commit suicide.
I had a C&P exam and the examiner states how in the hell they have me rated as low risk for suicide when I have multiple attempts and all this crap going on.
My life has been hell, for no reason other than lazy, ignorant people. The VA in CT was pretty good, but still had problems. Denver needs to be closed, knocked down with everyone in it. They have a volunteer that plays the harp as you come in. Not one person realizes angels playing harps are at the gates of heaven. So every time I walk in, I associate the place with death. No idea if any of this is readable, but all part of the hell im in.

Mike, I want to help you scold, and to embarrass the VA into closely monitoring your failing and deteriorating health. I write this because I’m basically the same situation. And, after over 5 years of complaining, I finally made some adjustments to deal with the VA. I don’t know the final outcome of this, and I don’t know when or if the VA will get into a freaking mood that will again, jeopardize my health.
Mike, let be straight up about this and to lay it on the table. Based on your content, you haven’t even put a dent in stinging the VA on the ass. I know how you feel, I’ve been there, but now I grabbed ahold of a nice thick and fleshy VA buttock, and I’ve been purposely stinging the fuck out of them, in order to wake their incompetent asses up. At this moment, I’m in a hospital bed located in my living room. Not able to get out. I wasn’t like this when I first walked into the VA to obtain medical care. So this is why I’m concentrating and implementing strategies to possibly embarrass the shit out of the VA. Yes, I’m honestly being a thorn of a hornet in VA’s ass? So Mike, it took a lot of time to write my response, and please carefully review.
1) If you haven’t already, start a VA Medical Treatment Log. Write all contacts with any VA Staph. You’ll need this to help keep things in order, so that; you’ll be able to remember easier, and to be able to have other understand since you created a timeline (your notes condensed).
2) What ever areas of your health, do what you can do to help your life and health become better. Foods, supplements, nootropics, MMJ (Medical Marijuana). If you know, and can prove that the VA has failed, then Brother, treat your own self. If you use MMJ and it helps you, continue to use the MMJ, isolates, waxes, other forms, and also CBD oil. With CBD, you don’t need a MMJ Medical Card.
I’m not only talking about pain relief, I’m talking about constipation issues, and other things that has been caused from taking Opiates.
3) The VA extremes hates negative publicity. You’re going to need to stand up for this one. And, it will really stir things up in the halls of your local VA’s Medical Center. Ready, well you will need to tell the VA that you KNOW and BELIEVE that they have been treating you negligently, your condition has become worsened, your PTSD is out of control (PTSD was never like this before), and that you know that your life is in danger, due to your failing health from all of the contradictions, lies, manipulation, and bullshit that the VA has been putting you through.
In other words, I want to encourage you to get in the VA’s face, with reason that is. That’s why you’ll need to create a timeline that reflects you medical care from the VA. And, if you haven’t kept a timeline, then start one right after you read my post. This tool is very important, and it will help you to keep everything in perspective.
4) Start gathering yourself a support team. Tell those you trust and who care about you, what is going on. Then directly ask them to help you with emotional support. If they can help you with finances for you to get vitamins, food stuffs, or other so called alternative help for your health, this type of health will be a plus to maintain what health you do have (no more slipping into the abyss).
When you share with your supporters about what you’re going through, make sure that you directly share with them, that this has you on edge, and that you really need them to stand by you. Get them to say that they’ll be on your side. Why?
Don’t assume anything. Some individuals are fucking narcissistic opportunists who NEVER say that they are sorry (because their shit DON’T stink, and they NEVER do anything wrong). Plus, these types of ass wipes, will take advantage of you because your down and sort of out, and that your really concerned about your life.
Mike, for anyone to really give a correct and right on assessment, they need to see you face to face, to be able to evaluate you, and to observe you while trying to perform certain medical diagnosis tasks, so that the Medical Professional can make a half-way decent Medical Report about you.
So please don’t entertain those who make their ignorant comments. It is healthy to eliminate these types these types of people out of your life. Especially if they show no signs of changing, or they don’t want to change. Their whole objective will be to keep you down, try to beat you up with their off-the-wall non-professional psychological commentary, and inform you that it is because of you, that things aren’t changing. They get offended and tired fast, and can’t truly hang in there with you, as a human being or so-called fucking friend.
These types of people truly don’t understand, but they’ll brag by saying that they do understand, give you a sermon, then label you with false tags, and leave you to solve your issues all by yourself. Enough about the fail out of those who can’t hang with you until you see some different results.
As you know, these types of people are all over the place. And, they may even by family members. You need people that is willing to back you up. In the good times and the bad, with no time limitations set on you, until you’re able to show positive signs of recovering that relate to your current medical condition.
5) He’s a method that will hopefully add to your Personal Support Staff. If you’re diagnosed as being disabled no matter what age, each State usually has program that is designated to help those with their health care, issues and problems, and, this is important, they can be you Patient Advocate. You’ll need to find a State Health Care Ombudsman to hopefully locate a State Patient Advocate that helps those with disabilities. You may want to start looking under your State’s Aging and Disability Services that are available.
Your goal is to find an Advocate that you can tell about your problems in receiving medical care from the VA. Mike, focus on that you don’t trust the VA anymore, that you believe and know that your health and life is in danger. Plus, inform them of the PTSD, and that you don’t want ANY VA Staph to touch you. These are trigger points that tells the Advocate that you need help.
Mike, I realize that this shit can be frustrating because of repeating yourself all the time, people forgetting what you shared with them, questioning the VA’s Medical Practice and Modalities that they are treating you with per you all over the human map medical diagnoses. You want to really sweet talk the State Advocate or Ombudsman. You REALLY need someone like this on your side. Why? Because they will now become your over seer, your mouthpiece, your battler against the VA, and by having a non-VA Representative in your support group, this will reduce your stress tremendously. You’ll feel this when you get a State Advocate on your side. I felt it when I got mine.
6) Find a VA Medical Malpractice Attorney (use this as a search phrase in Google to find one). Now, since it seems that your medical condition has been one that has accumulated over time (important), you’ll need to share this with the attorney’s. But first, you must complete the medical care timeline that I mentioned earlier. You will then be able to share the timeline with the attorney. Make sense? I know it does.
I say this, because nowadays like many, malpractice attorneys are looking for the easiest cases to take on (botched surgeries, wrong medication taken for a long time or one that hurt you in some way). These attorneys are looking for easy things that are straightforward, where they don’t have to spend a lot of time in investigating. The timeline will help the attorney to see that over time, incompetent care, has worsened and deteriorated your health and well being.
In conclusion, the VA is set up to deny, deny, until you die. Why do I say this? Because just by the amount of Veterans that the VA has turned down in their claims, and the influx of Veterans that use the VA, this Federal Agency wasn’t prepared in taking care of us Veterans who need help. Therefore, even if the VA accepted claims that were scientifically proven to be valid, this would totally bust the VA’s budget. But yet, they still pay out those bonuses to the AFGE VA Employees.
But don’t let this discourage you. Keep on doing things that will help your cause. Don’t listen to others that have give you negative comments. Mike, I hope this helps some. Each Veteran is different. So that’s why it is good to look over suggestions individually. Use what you need, or make adjustments. Do things that will move you forward towards the goal of establishing a support group. Then lay out your plan of attack (sorry sounds negative but its reality), and create how you’re going to see your situation through for changes. Good luck. – – – Nutter.
I like the process you described. My daughter has been fighting the VA for a couple years. Appointment date mail would show up week to 10 days after the appointment date. Or they would give her 10+ appointments within one week all of those notifiers show up after the date. I have seen the letters and cards. She has a pissy job and can’t afford to loose it and she would if she made a few of the appointment.
Talking to the patient advocate from the VA is of no use and none of the Representative give a shit.
They put her of Prozac, have no idea why, but she has gained about 100lb since then.

Everytime I read this blog I get angrier and more depressed. But I keep reading because it is a cross between watching a train wreck, (and I’m in it) and finding those who are just as mal-treated and pissed off as I am. The last week after not believing the VA dermatologists anymore I went to a non-VA derm. Instead of the one cancer on my face they said I had, there were four plus one on my breast. This is after a year ago them telling me it was psoriasis and giving me a cream. Now it is a big open bleeding sore. I will have at least a two inch scar on my face which could have been avoided. Maybe that is a handsome rugged look on a man but not on a woman. But no one would wish to have a scar on their face, especially over bullshit. The other three spots on my face were also dismissed by the VA as psoriasis. So what, next year I can have three more two inch scars on my face? Why?

Ben’s article highlights a systemic problem at va hospitals, local facilities failing to comply with national directives. You can pick just about any clinical or non-clinical program and there will be non-compliance in some form or fashion. I’d like to think these non-compliance issues are unintentional but the frequency of these reports is truly disturbing. Until the va recommits itself to the mission statement, us veterans are subject the va’s errors and omissions, often with no recourse.

So what we need is to setup and fund a Veterans organization that is willing to go to war with the established VSO’s and expose them for the crooks they all are. We then can sever the “relationship” the VSO’s have with the VA. Once we have numbers and strength we can then start going after the VA.

@Wyldechylde – – – You’d think that from our military experiences, then applying those skills and capabilities that were learned when serving, that many Veterans would be more than willing to form a group, that will focus on certain VA issues, that are in need of reform or reorganization. Many Veterans are reluctant in joining other Veterans, to investigate certain areas of the VA, may be because of suspicions, lack of trust, and other areas of social demarcation points per each individual Veteran. And, some Veterans at the present time, are tied up or overwhelmed is solving their issues with the VA. Whether it is for medical care or the claims processes. My self, I always thought that many Veterans would gather for one cause; help to put pressure on the VA to fix? (the way @Dennis and others feel)
For me, I’m focusing on gaining as much of my life back as I can. Due the VA’s actions, or inactions, I shouldn’t be in this condition. I’m very close to being admitted to a non-VA hospital. I’m homebound, the VA provides me with a visiting Nurse, that sees me once a week for an hour per visit. And, still there is no treatment plan for me. Conclusion; waste of money and my health.
When things get stabilized, I plan on doing what I can, or joining a certain group, that their mission is to expose the VA. Expose them first, then apply persuasion pressure on them. What a job this will be. But until then, I’ll keep updated on Ben’s Blog.

this happened to many Viet Nam vets in the 70s and 80s. But nobody paid the price for that. Their applications ended up in the trash and shredded. Vets in many cases have trouble existing in our society as it is, leave alone cross all the T’s and dot the I’s.

In it, “Lying Shithead Shulkin” states, (paraphrasing) he doesn’t understand why veterans are committing suicide in his VHA parking lots. He further says he believes veterans do it there because VA will take care of the veterans properly! Therefore the family won’t have to discover the Veterans body!
There’s also a portion of the article about civilian healthcare and the opioid crisis!
Y’all have got to read the bullshit Shulkin has come up with concerning the “Suicide Crisis” at VHA’s Nationwide!
Even when Shulkin is presented with the “reason” why a vet commited suicide, he still bullshits his way out of it!

Stupid shit from all over actually. Yes the article is a pisser and Shulkin is, again, another one, typical highly paid scum and professionals at lip service and propaganda. But, the corruption and liars are too many to count, and too many groups and companies, reasons, to list.
Local news fails to report much real news. IF they do they have their usual lefty spin on it. They refuse to report veteran suicides or any suicide for that matter unless it happens in public and are forced to say something about it. So we won’t hear about others issues locally. Too much censoring and propaganda for them to do. If we do hear of such events in the local grapevine like someone stepping out in front of a semi truck or sitting on train tracks, etc., we have to come to our own conclusions or what the family might report to someone. Indiana is not known for decency, transparency, integrity, real news, real community events/happenings, or truth.
I read the Indiana Business Journal among others. It hints and tells who all effects our care and care from the VA. Or to why we vets and Americans can’t get what we expect from the corporate/college/unions/political complexes or cluster F’s.
In Indiana if you’re a vet and left the VA it means being investigated and waiting. Ran through checks for doctor shopping, pharmacy checks and prescription details from all over. Told we go through criminal record checks through Homeland Suckerity to see what can be dug up if anything. Then the drug screens, again. Then more games I will not play like sitting in pain clinics for six hours for pain meds. Can’t be done.
Have been checking and the doctors I ‘ve been told about that do give pain meds, internalist to pain clinics, ask about my insurance which is basic Medicare and cash, turn me down, but accepting people I know who are on medicaid or those with illegals living with them (the illegals) who can get pain meds from special foreign doctors located here. I call the same foreign MD clinics and told they are not accepting new patients or Medicare basic. But I have seen illegals – can’t – get that care or social services here, yet they claim we are a sanctuary city, and they are being taken care of. I have had to wait with vets trying to get them into the emergency ward while the hospital staff scurry them (illegals) into another section of the hospital behind closed doors. Go figure. I can’t get what they can. Indiana does not have Supplemental Medicaid for us as other states provide, or insurance is allowed to provide, just shy of a few months of turning 65 to try to get that going.
The opioid claims from him is Bull too. No it is not easier to get pain meds or the strength of muscle relaxers needed in civilian care. Or get decent care. I was told the VA and state, civilians, are going by the same legislation state, and medical boards rules for dispensing pain meds. Civilian care is just as messed up as VA care. Complete with misplaced files, Xrays not worthy enough to read, games, contracts, etc. Bottom line is, the opioid change and new demands were in place over-night. If the VA, universities, med schools, law makers, scum, liars, corporations, media, AFGE, service unions, etc. wanted real change it could and would happen just the same way…. over night. The whole damn sheebang in DC and on local to state levels with more added to the list needs fired or their medical and teaching license needs ended.
PS. Civilian care does not want or like going through the fifty pounds of paper work the VA sends out to them either. They don’t bother about learning about our issues that much. All the clinic appointments and such are interspersed with all the others and makes it a mess to find anything. Plus the lost tests that aren’t included or they claim they want their own. They all want their own test…money money money.

Depressing. I think the statistics belie another reality. VA Hospitals are easily accessible for desperate vets who are on the verge of ending their lives. The common perception is that the suicides are statements, or otherwise a manifestation of delayed healthcare. However, with 65% of those suicides being committed by those who are age 50 (I’ve heard 55) years and older, it may suggest another motivation. How many of these casualties take place on VAHC grounds because the vet couldn’t get to or find the ghost VA benefits office that have denied them their claims for the last five or ten years?
It was that way when I came back from Nam in 69. There was no information to be gotten regarding VA care from anyone.
Even now it’s virtually Impossible to get info and Rx services in a timely manner. I visited my daughter in Baltimore 8 months ago suffering from excruciating headaches and tried to get some meds from the Baltimore clinic. After I waited for 4 hours they took my BP which was 235 over 112. Even than I was still waiting while street drunks claiming to be vets and could not be identified took precedence over me.
I left and went to an Emergency unit were I was given an injection and kept for some time for observation. Veterans health care is a bigger rip off of the public taxes than anything else.
To guy before your post. All is well and still bullshit. Though allow shulkin to continue the list of firings, only relief of injured or dead. We will now call it Shulkins List. Up until (approximately) 2005-6, 2014 and again in 2015, the VHA in Lake Baldwin, Orlando, Florida, did NOT have my Army service records, or so they claimed. Therefore, my “Boots on Ground” Vietnam Duties was NOT listed! They only had my Naval service recorded. Since 2005 or 06, I thought I had “fixed the problem”! Not so. In 2014, and again in 2015 and 16, the wife and I had to correct misinformation in my records!
This, in some ways, explains why the medical field was reluctant to diagnose me with conditions relating to my Army overseas “Combat Duty!”
This, some might think, is standard practice at VA’s nationwide! I think it is! Because, IF VA can figure out a way to screw the average veteran, they will!
Here’s the kicker – The VA has not only had my Army DD-214’s since 1971 or 72, well before my Navy discharges, they’ve had my Army Medical Records for the same time frame!
It’s been a real challenge to insure my FULL dual-military service, both branches, stays on the books at VA. They’ve changed my military service multiple times. They’ve changed my address multiple times. They’ve changed my marital status multiple times. They’ve changed my wife’s address and marital status multiple times. ~It’s a never ending job keeping VA employees honest!~ Evidently, “record keeping” is NOT in VA’s educational system!
So, reading Ben’s “blog” today, is not a surprise for me and/or my wife!

These “local policies” allowed at each VAMC that may differ markedly or only slightly, collectively, makes the VA a systemic mess. Notice how the VA OIG used the word ONLY in that “…ONLY 38 of 106 medical facilities had local enrollment policies…]”, as if 38 out of 106 was a GOOD THING? If you took a college course and you missed 38 out of 106 questions, you would be lucky to get an even 60%, which is a FAILING GRADE, rat bastards!
With absolutely no adherence to STANDARDIZATION across the VA, you end-up with a SYSYEMIC CLUSTER FUCK…and I am now convinced this is the way the VA prefers it as they have proven over and over again that they have ZERO INTEREST in complying…ZERO.
Since the VA has recently redefined accountability and what it does not mean for the VA, you can not convince me that nobody in upper management or elsewhere cannot see the systemic mess created from remaining stubbornly a systemic mess…and the irony of all this that shoves that boot even further up Veteran’s asses is FACT we Veterans are TRAINED to operate and thrive within a well-oiled system. With that said, the VA KNOWS they are a systemic mess and it’s all part of their psyops in fucking with us.
